AYSO Region 112 Withdrawal and Refund Request Form
To withdraw your player from AYSO Region 112, complete this form.
--One completed form is required for each player being withdrawn.
--Withdrawal is effective immediately upon submission of the withdrawal form.
Refund Request:
--A portion of each registration is for a $25 National Player Fee (NPF). If the $25 National Player Fee (NPF) was paid, it is non-refundable and will be deducted from all refunds unless noted in list below.
--The credit/debit card processing fee is non-refundable.
--A child may not play in AYSO games or practices once a withdrawal form is submitted.
--Refunds are normally processed within 2 – 3 weeks.
Season refund deadlines:
--Before the First Game: You are eligible for a full refund, less the AYSO National Player Fee (NPF) and the non-refundable credit/debit card processing fee.
--After the First Game: Once the season begins, no refunds will be issued for players placed on a team.
--Unallocated Players: If we are unable to place a player from the paid player list, a full refund (including the NPF) will be issued; less the non-refundable credit/debit card processing fee.
Refunds are contingent upon the return of a clean, lightly used uniform. If a uniform or other Regional equipment is not returned, any refund remains at the sole discretion of the Registrar or Regional Commissioner.
If your refund request is denied due to timing, you may be able to claim the registration amount as a charitable donation to AYSO on your tax return.
Refunds are issued directly to the original credit card used at checkout. If the card is no longer active or the transaction cannot be processed, a refund check will be mailed to the original registrant.
