AYSO REGION 112 WITHDRAWAL AND REFUND POLICY

IMPORTANT:  TO WITHDRAW YOUR CHILD FROM AYSO REGION 112, COMPLETE AND SIGN THE AYSO REGION 112 REFUND REQUEST FORM.

• One completed form is required for each child being withdrawn
• Withdrawal is effective immediately upon submission of the withdrawal form.
• A child may not play in AYSO games or practices once a withdrawal form is submitted.
• Refunds are normally processed within 2 – 3 weeks.
• Season refund deadlines:
- Before the first game, you will receive a full refund minus the AYSO National Player Fee (NPF) and Processing fee.
- No refunds will be given after the first game has been played.
• In order to protect your rights, all refund requests must be submitted in WRITING.  Phone calls and e-mail are insufficient to process requests.  If a refund request is denied due to the date of the request, you may deduct the amount from your tax return as a donation to AYSO, a non-profit organization.
• If Region 112 is unable to place your registered child on a team, you will automatically receive a full refund including the AYSO National Player Fee (NPF) minus the AYSO National Processing fee.
