If we do accept a player's registration submitted after that date, the regular season fee will be determined and, depending on the program's needs, the player may be unable to play before the third or fourth week of the season.

If you move or change your phone number, please mail or e-mail us the new information. E-mail should go to registrar

VOLUNTEER PARTICIPATION

PLEASE FILL OUT THE VOLUNTEER FORM. BE GENEROUS WITH YOUR TIME. ALL OF THE CHILDREN WILL BENEFIT FROM YOUR INVOLVEMENT.

TEAM ASSIGNMENT/PRACTICE AND GAMES

Team assignments will be conducted in late July. Once AYSO has assigned a team to a coach it is the coaches' responsibility to contact each player. Please record the name and phone number of your coach and division director. DO NOT CONTACT US BEFORE SEPTEMBER 1. There may up to two weekly practices, one mandatory, and one or two games per week. Punctuality is required for practices and games.Team assignments are final. Coaches cannot change player's team assignments and they cannot add players to their teams. If a player drops from your team, contact the registrar for possible players on the waiting list. (Violation of this rule will result in immediate removal from any involvement in our soccer program.) We reserve the right to adjust teams to maintain team balance. Team assignments are not based upon religious school schedules, car pool needs or any criteria other than achieving balanced teams.

Kids on the Waiting List

If your child is on a waiting list there is no guarantee they will play. However, if the registrar calls you to place your child on a team, please have your mind made up as to whether or not they are going to play. Many parents when called then take a further week to decided on their child playing. This causes several problems for us as an organization. So please, make your decision on your child playing if you are waiting on a list. We do everything we can to ensure every child plays.

REFUNDS

Players withdrawing prior to July 1, 2008 will be issued a refund (minus a $15.00 processing fee). After July 15, 2008, players withdrawing will receive a refund of 50% of fees paid, minus a $15.00 processing fee.

There will be NO REFUNDS at all after September 1, 2008. In order to protect your rights, all refund requests must be submitted in WRITING. Phone calls and e-mail are insufficient to process requests.

ALL REFUND REQUESTS MUST BE MADE IN WRITING AND INCLUDE A SELF-ADDRESSED
STAMPED ENVELOPE.

PLEASE MAIL YOUR WRITTEN REQUESTS TO:
AYSO REGION 112 REFUNDS TREASURER/REGISTRAR
P.O. BOX 264, SAN DIMAS, CA 91773

Include the player's name and date of birth, along with the reason they are withdrawing. All uniforms must be turned in to the coach when you notify him/her that you are withdrawing. Soccer requires a minimum of 2-3 hours of practice per week, plus a Saturday game of 1-2 hours. Please consider your child's schedule and abilities. It is a disservice to your child's team if you withdraw after the teams are formed and balanced. Please decide early what your family schedule will accommodate. If a refund request is denied due to the date of the request, you may deduct the amount from your tax return as a donation to AYSO, a non-profit organization--please ask your tax advisor about this."

If you register a child and then decide for any reason that the child will not play in our region, please notify us as soon as possible. If Region 112 is unable to place your child on a team, you will automatically receive a full refund.

Proof of Age

Children who did not play in AYSO Region 112 for the Fall 2007 season and are between the ages of 4 1/2 to 18 as of July 31, 2008, will need to bring a copy of their birth certificate and medical insurance information in order to register their child to play.
